Before any placement, we carry out a detailed Initial Needs Assessment and Risk Assessment to understand the individual’s mental health history, current needs, behaviours, risks, and aspirations. This enables us to:
- Identify appropriate levels of support
- Ensure a safe transition into the placement
- Put in place proactive and preventative strategies
- Design a bespoke care plan from day one
We regularly review and update these assessments to ensure support remains relevant, safe, and effective.
Mental Health Support
We support individuals with a range of conditions, including:

- PTSD and complex trauma

Personalised Care Planning
We co-develop person-centred care plans that reflect the individual’s specific needs, choices, and goals. These plans are reviewed in partnership with the person, their family (if appropriate), and professionals involved in their care.
